College Disc Golf Roster Form Instructions

FOR STUDENTS
Print the entire form (all pages).
Complete all required information (marked with an asterisk *), including the roster table.
Submit the completed form to your school registrar’s office for verification.
For Registrars
Please review and certify that each student listed meets the eligibility requirements:

The student must be enrolled in a degree or certificate program at the school they represent.

The student must be enrolled at least half-time (minimum of 6 credit hours or equivalent).

Graduate students in research, teaching, or thesis programs qualify if the program leads toward a degree.

Eligibility must be maintained throughout the semester or academic term.

For quarter-system schools, requirements apply for at least two of the three quarters (any combination of fall, winter, and spring).

The half-time requirement is waived for students who are:
-
(a) taking all remaining courses required to graduate, where “remaining” is at least one class, and
-
(b) on track to graduate at the conclusion of the winter or spring term of the current academic year.

Please also provide a copy of the roster to the student who submitted it.

Note: Teams must include stamped instructions with the roster. Do not accept or stamp the roster without the accompanying instruction page. College Disc Golf strictly enforces eligibility verification for all teams.
